Practice Support consists of engaging relationships with providers aimed at customer service through researching and resolving provider issues, facilitating training opportunities, and providing high-quality coaching within practices. Our Practice Support also recruits practices and providers for an array of programs that align with Partnership for Community Care and DMA visions.
Quality Improvement is a systematic approach to identify opportunities for improving systems and processes that ultimately lead to improving outcomes. Our Quality Improvement Department provides quality tools such as PDSA and Lean strategies, resources such as PCMH Content Experts, and data to practices to assist in improving patient outcomes, reduce cost, improving practice systems and processes, and patient population management. These constructive solutions to identified barriers are designed to help improve the effectiveness and efficiency of the staff and practice in their delivery of healthcare services.
