Referrals for Care Management services are received in many ways:

  • Care Managers receive daily notifications of patient’s with CA II Medicaid who have been to the ED/local and out of network hospitalizations
  • A Primary Care Provider can refer a patient for specific issues
  • A patient can refer themselves
  • Patient Priority Lists Each provided by CCNC via our Informatics System where we can find patients w/ specific disease diagnosis/over utilization of services.

We treat the whole patient medically, mentally, and socially. Patient comes up with patient centered goals that are reviewed every 30 days.

Note: The patient must be currently enrolled as a Medicaid CA-II recipient. The patient also must be notified of the referral and to expect a contact phone call from a representative at Partnership for Community Care.
